Our Health care team consists of:

The health care team works together as an
interdisciplinary team to provide the highest quality of care for
our residents. At least every 90 days a comprehensive review is
made by the Health Care team to assess the resident’s ongoing plan
of care.
The resident and family is invited and
encouraged to attend the Plan of Care meeting. The resident and
family provide valuable information that is implemented into the
plan of care. |
